SBI Foundation’s Jivanam Sanjeevani Program in India

The SBI Foundation invites non-profit organisations registered in India for implementing projects in the thematic area of primary healthcare through its model in healthcare.

Thematic Area

• Providing quality primary healthcare services to the most underserved/unserved areas.
• Framework of SBI-Sanjeevani
  • Obtaining necessary approvals from the local health department.
  • Office set up in the selected project area.
  • Purchasing mobile medical/ medical equipment, branding & fabrication of van.
  • Recruitment of medical team.
  • Health profiling of the selected villages.
  • Maintaining the patient data physically/digitally.
  • OPDs will be conducted in selected villages (As per the journey cycle plan)
  • In referral cases the medical team will ensure the follow-up treatment of the patient either in nearby PHC or hospitals.
  • Community awareness sessions will be conducted on a monthly basis on various health topics
  • Specialized health check-up camps will be conducted every quarter.
  • Waste Management/Swachh Bharat Abhiyan Initiatives will be conducted quarterly.
  • Process documentation and periodic submission of consolidated reports.
  • Final evaluation/Impact assessment report

Funding Information and Duration

• The proposed budget for the proposal should be in the range of Rs.95.00 Lakh to Rs.1.90 Crore for a period of 15 months.

Eligibility Criteria

• The NGO should have a minimum 3 years of registration.
• The NGO should have a minimum of 3 years of work experience in the proposed state.
• The NGOs having demonstrated capability to improve measurable outcomes to ensure effectiveness and efficiency of the intervention.
• The Agency should have previous experience of executing projects in provision of primary healthcare services in remote areas.
