The purpose of this NOFO is to continue the acceleration of sustainable, high-quality comprehensive HIV prevention, care and treatment services, to achieve the 95-95-95 targets and HIV epidemic control, and transition to a sustainable service delivery model.
Outcomes
CDC may require or allow applicants to propose additional related project period outcomes other than those identified in the NOFO.

Increased uptake of HIV prevention services among KP, PP and GP
Increased uptake of comprehensive evidence-based interventions among AGYW and OVC
Increased uptake of VMMC services
Increased ART uptake among HIV-infected PBFW and ARV prophylaxis and EID among HEI
Increased uptake of efficient, cost-effective & innovative HIV testing & linkage strategies
Increased uptake of comprehensive client-centered ART services among all populations
Increased access to facility and community TB/HIV services
Increased access to HIV and TB laboratory services, and sustained biosafety standards
Improved data quality and utilization; and uptake and integration of HIS
Increased capacity of county governments to oversee and implement HIV services
Intermediate Outcomes
Increased coverage of HIV prevention services among KP, PP, AGYW, OVC and target populations
Reduced HIV-related vulnerabilities among KP, PP, AGYW and OVC
Improved retention of HIV-positive PBFW, HEI, and reduced MTCT of HIV
Increased knowledge of HIV status and linkage among PLHIV
Improved ART coverage, retention and durable viral suppression across all populations
Improved TB case detection, prevention and treatment success
Improved quality of HIV/TB diagnosis and treatment monitoring
Improved data-driven planning and decision making
Improved HIV program oversight and funding by county governments
Long-Term Outcomes
Reduced incidence of stigma, discrimination, and violence among KP and PP
Virtual elimination of MTCT of HIV
Reduced HIV and TB related morbidity and mortality across all populations
Increased investment and accountability of county in HIV/TB program
