The Department of Health & Human Services and State Government of Victoria is inviting proposals for the 2021-23 Health Condition Support Grants Progam to provide grants that assist Victorian peer support groups to meet some of the costs associated with providing social, emotional and practical support to members living with a specific health condition.

For the purpose of the Health Condition Support Grants program, a health condition is considered to be a long-term condition with persistent effects, ranging from mild to more significant conditions. Examples include, but are not limited to:

chronic kidney disease
arthritis, osteoporosis and other musculoskeletal conditions
respiratory diseases (asthma, chronic obstructive pulmonary disease)
cardiovascular conditions (coronary heart disease and stroke)
diabetes.

Peer support groups that demonstrate that the grant will assist in the delivery of one or more of the following benefits for their members:
sharing of both formal and informal information such as personal experiences and coping strategies to empower people to make informed decisions about their condition
providing a social context for people who may otherwise be isolated
providing empathy by allowing people to express their feelings in a supportive environment
fostering hope for members who struggle with their condition and its limitations on daily life
encouraging behaviour change to minimise and manage symptoms.
Peer support groups that demonstrate that the grant will assist them to strengthen the group’s ability to:
increase the number of people they are supporting
build capacity of the group to become more sustainable into the future
improve their operational effectiveness.
Peer support groups that demonstrate that the grant will assist them to:
increase participation from Aboriginal people
increase participation from people from non-English speaking backgrounds.
Peer support groups that have not previously received a grant from the Department of Health.
Funding Information

Grants of up to $5,000 per annum are available for a two-year period to peer support groups focusing on a specific health condition, that provide mutual support and education to members and increase community awareness of the health condition.
Type of Activities
supporting groups to increase their online presence and transition to online distribution of information (including, website upgrades/development, video streaming)
increasing the peer support groups membership base (including, resources to support groups having a presence at local events, promotional materials in other languages or supporting Aboriginal or culturally and linguistically diverse communities)
supporting information and communication distribution (including, printing / printing supplies —including paper and toner refills — for brochures, posters, newsletters, and information booklets)
increasing members’ access to latest information relevant to their specific health condition or managing their health condition generally, such as information on goal setting techniques (including, reference materials)
general overheads and support for face-to-face meetings (including, venue hire and interpreters)
improving the operational effectiveness and efficiency of groups (including, training and development).
Eligibility Criteria

To qualify for a grant, a peer support group must:
focus on a specific chronic health condition or disease
be led and managed by members who themselves have lived experience of the health condition
be a non-government and non-profit making group at the time of application and for the project duration
possess an Australian Business Number (ABN) or provide a completed Australian Tax Office form (Statement by a supplier) or be auspiced by an organisation that has an ABN
have a bank account in the name of the peer support group or be auspiced by a not-for-profit organisation that has a bank account and both parties have signed the letter of agreement
have satisfactorily met reporting requirements on any grants previously received from the Department of Health.
